ACO Care Coordinator - SWHR CIN - Dallas
The ideal candidate will possess the following qualifications:
• Graduate of an accredited vocational nursing program required Or,
• Master’s Degree in Social Work required.
• 3 years related clinical experience required.
• LVN – Licensed Vocational Nurse upon hire required Or,
• LMSW – Licensed Master Social Worker upon hire required and
• Current auto insurance upon hire required.
Skills:
• Work requires ability to maintain current customer service, clinical, and technical skills by attending training programs and seminars to continue level of competency and ensure compliance with departmental policies and procedures.
• Work requires knowledge and ability to adhere to department policies and procedures.
• Work requires ability to work independently and determine methods and approaches to patient issues.
• Work requires excellent communication techniques and well developed communication skills.
• Work requires the ability to organize, coordinate, and demonstrate judgement, diplomacy, assertiveness and initiative.
• Work requires frequent departmental interfaces and interfaces with internal and external customers to coordinate and provide optimal level of patient care.
• Work requires ability to lift, push, and/or pull up to 50 lbs.
• Work requires ability to read, write, and perform mathematical calculations.
