Work location:  Southwestern Health Resources Headquarters, 1601/1603 Lyndon B. Johnson Freeway, Farmers Branch, TX 75234.

 Travel within the DFW metroplex

Work environment:  Care Coordination Department.

Please note:  The Clinical Navigator - Care Coordination will be an employee of Southwestern Health Resources (SWHR).   SWHR is a clinically integrated network formed by Texas Health Resources (THR) and University of Texas Southwestern Medical Center (UTSW). 

Work hours:  

·       Full-time, 40 hours per week.

·       Monday - Friday, Day shift.

·       This is a Hybrid position, requiring onsite workdays during the week, based on department business needs.

·       In-person attendance required at department meetings, trainings and/or other department authorized activities, at the above-mentioned address or other locations as directed by the department management.

Education
Bachelor's Degree Social Work Req Or
Associate's Degree Nursing Req
Master's Degree Nursing or Social Work Pref

Experience
3 Years Clinical care management or post-acute experience. Req

Licenses and Certifications
RN - Registered Nurse Upon Hire Req Or
LBSW - Licensed Bachelor Social Worker Upon Hire Req Or
LMSW - Licensed Master Social Worker l Upon Hire Req

Skills
Analysis and critical thinking; using resources available to solve problems and research solutions independently.
Strong interpersonal skills to include effective verbal and written communication.
Solid time management with the ability to prioritize multiple tasks.
Ability to collaborate across various levels of management, departments, and teams.
