Southwestern Health Resources (SWHR CIN) seeks to hire a Patient Experience & Clinical Safety Specialist to work Full Time supporting the Patient Experience & Clinical Safety team.

Salary range is Min $30.33 - Max $48.51 – based on relevant experience
 
 
Job Description
  • Assists manager and director (team leadership) with performance monitoring and improvement plans; ensures feedback loops and monitoring systems related to clinical safety and patient experience are functioning as intended.
  • In collaboration with key stakeholders and senior management; defines project scope, aims, deliverables, and timelines for SWHR; develops full-scale feasibility (including resources) and charter documents with associated communication documents; communicates effectively the project plans at committee levels; communicates project expectations to team members and stakeholders in a timely and clear fashion.
  • Leads and coordinates delegated project related activities between the SWHR staff, department leaders and committee chairs to promote effective communication; communicates to SWHR leadership any resource needs; identifies barriers to project implementation.
  • Manages Rapid Cycle Improvement strategies in response to particular clinical safety events; delegates tasks and responsibilities to appropriate team members; identifies and resolves issues and conflicts within the project team, with input from SWHR leadership, when necessary.
  • Generates and manages project timelines and milestones using appropriate project management tools; tracks milestones and deliverables identified in charter; creates and disseminates progress reports and required documentation for regulatory purposes; determines frequency and content of status reports.
  • Analyzes and monitors Patient Experience data and other identified key metrics (CG-CAHPS and contractual requirements) to drive improvement outcomes for Patient Centered Excellence; promotes excellence in care through ongoing evaluation of data; communicates and collaborates with colleagues on opportunities for improvement and best practice.
  • Conducts analysis and prepares reports on clinical safety events and patient experience outcomes as requested by SWHR leadership; identifies opportunities to improve patient safety; identifies trends and patterns related to near miss and actual events; develops conclusions and makes recommendations through data presentations to SWHR leadership, committees, and teams.
  • Assists with FMEA (failure mode effects analysis) and RCAs (root cause analyses).
    Analyzes data and results to determine areas for improvement through the project implementation and during the maintenance period; utilizes analytic tools necessary to delineate accurate results and drilldown.
  • Prepares reports and analysis of clinical safety events and audits data in order to identify opportunities for clinical process improvement initiatives.
  • Implements continuous performance improvement using Six Sigma or Lean methodology, based on the needs of the project.
  • Prepares written and oral presentations for a variety of internal and external audiences.
  • Prepares summary of results in appropriate format to report to management and Safety Committees.
  • Monitors clinical safety trends in healthcare industry and suggests program changes or enhancements as appropriate.

The ideal candidate will possess the following qualifications:
 
Education
  • Bachelor's Degree in Healthcare related field Required
  • Master's Degree Preferred
 
Experience
 
  • Bachelor’s Degree Required And
  • 3 years’ experience in Healthcare Delivery Required And
  • 2 years demonstrated knowledge in Clinical Safety/Patient Experience using structured Process Improvement methodologies (i.e. Lean, Six Sigma, RCA, FMEA) Required And
  • Quality Improvement/Practice Improvement (QI/PI) and/or Clinical Safety/Patient experience with a Healthcare focus Required
 
OR
 
  • Master’s Degree Preferred And
  • 3 years’ experience in Healthcare Delivery Required And
  • 1 year demonstrated knowledge in Clinical Safety/Patient Experience using structured Process Improvement methodologies (i.e. Lean, Six Sigma, RCA, FMEA) Required And
  • Quality Improvement/Practice Improvement (QI/PI) and/or Clinical Safety/Patient experience with a Healthcare focus Required
 
Licenses and Certifications
  • Specialized Certification in Clinical Safety and Patient Experience (i.e., CPPS, CPXP, etc.), obtained from a nationally recognized organization Upon Hire Preferred Or
  • PMP - Project Management Professional Certification Upon Hire Preferred Or
  • Other Clinical licensure Upon Hire Preferred And
  • Certification Upon Hire Preferred or within 1 Year of Hire
 
  • Basic Data Analysis experience Preferred
  • Ability to utilize and be proficient in Microsoft Suite, primarily in Excel and Report Creation Preferred
  • Effective oral and written communication skills
  • Excellent customer service
