Population Health Service Company - Post Acute Clinical Specialist - SWHR CIN - Dallas
The ideal candidate will possess the following qualifications:
• Associate’s degree nursing required
• Bachelor's degree nursing
preferred.
• 5 years experience as a Registered Nurse in acute care setting and/or
transitions coaching, care management, or patient triage required or
• 7 years skilled Nursing experience required and
• Emergency Department, Intensive Care Unit, or Progressive Care Unit preferred.
• RN – Registered Nurse upon hire required
• Drivers License upon hire required
Skills
• Expert clinician with highly developed skills in the 5 C’s of nursing.
• Subject matter expertise in disease management: sepsis, cardiac,
pulmonary, diabetes, wound and orthopedics.
• Broad understanding of level of care environments including acute
hospitals, post-acute facilities, home health, DME,
outpatient services, and PCO clinic
operations.
• Works collaboratively with supporting the ACO quality performance
targets and measures addressing gaps in care and service as it relates to care
management.
• Excellent intrapersonal skills, ability to communicate and articulate
both verbally and written.
