Southwestern Health Resources is seeking to hire a Post-Acute Clinical Specialist to support our Population Health Services Company.

Salary range is $35.91/hour – Max $50.40/hour – based on relevant experience.

 
Work Schedule  

•  Full-time: 40 hours per week, Monday-Friday

•  Field based position with 50% travel within DFW.

 

The Post-Acute Clinical Specialist (PA-CS) is responsible for managing the Population Health Services Company (PHSC) attributed population of high risk and/or complex chronic illness patients. Emphasis is placed on active patient/caregiver participation and collaboration with healthcare providers to improve transitions, prevent gaps in care, decrease exacerbations, readmissions, decrease cost and improve patient experience. The Post-Acute Clinical Specialist plays an integral role in optimizing the patient's recovery journey toward well-being. By serving as the link between patients and the appropriate health care personnel, the PA-CS is responsible for ensuring efficient, smooth, and prompt transitions of care. Using evidence-based approaches, interventions and best practices. Discerning the variation and complexity inherent in caring for vulnerable populations, including a capacity to evaluate and optimize available healthcare resources. The essential job duties for this position are:

 
• 
Collects pertinent data and information relative to the patient’s health or situation

•  Triage adverse events both clinical and non-clinical.

•  Identifies expected outcomes for a plan of individualized care.

•  Develops a plan that prescribes strategies to attain expected, measurable outcomes.

•  Collaborates with patient/family and other members of the healthcare team.

•  Performs in person (facility) and telephonic triage.

•  Provides patient education and disease management.

•  Advocates and facilitates access to services including referrals to community based resources and coordination of medical and social services.

•  Ensures patient knowledge and compliance with treatment.

•  Provides discharge planning and reduces barriers to discharge.

•  Implements health education programming in one-on-one and group settings to increase the knowledge and self-efficacy

of patients to prevent illness, manage health conditions, and improve overall wellness.

•  Serves as a member of an inter-disciplinary care team and promotes effective working relationships with internal and

external stakeholders.

  Establishes and maintains referral processes, resource lists, and other community networks to effectively link patients to needed health information and services.

•  Performs other duties as needed.


The ideal candidate will possess the following qualifications:


•  Associate’s degree nursing required

•  Bachelor's degree nursing preferred.
•  5 years experience as a Registered Nurse in acute care setting and/or transitions coaching, care management, or patient triage required or

•  7 years skilled Nursing experience required and

•  Emergency Department, Intensive Care Unit, or Progressive Care Unit preferred.

•  RN – Registered Nurse upon hire required

•  Drivers License upon hire required

 

Skills
•  Expert clinician with highly developed skills in the 5 C’s of nursing.
•  Subject matter expertise in disease management: sepsis, cardiac, pulmonary, diabetes, wound and orthopedics.
•  Broad understanding of level of care environments including acute hospitals, post-acute facilities, home health, DME,

outpatient services, and PCO clinic operations.
•  Works collaboratively with supporting the ACO quality performance targets and measures addressing gaps in care and service as it relates to care management.
•  Excellent intrapersonal skills, ability to communicate and articulate both verbally and written.
