Work Hours: Full Time Days (8:00am-5:00pm) for 40 hrs/week (remote work allowed at manager's discretion)

The Program Manager, Population Health supports the execution and ongoing management of population health and value-based care initiatives. This role works closely with the Director of Population Health Strategy to operationalize strategic priorities into actionable workplans, ensuring programs are implemented effectively, tracked consistently, and optimized over time.
The Program Manager is responsible for coordinating cross-functional efforts, managing timelines and deliverables, monitoring performance, and supporting continuous improvement activities. This role plays a critical part in translating strategy into daily operations that improve quality, patient outcomes, utilization, and total cost of care.
What You Will Do:
Lead the implementation and day-to-day management of population health and value-based care programs across the enterprise. 10%
Develop and maintain detailed project plans, timelines, milestones, and deliverables to ensure successful program execution.

Coordinate cross-functional teams including Quality, Care Management, Risk Adjustment, Pharmacy, Medical Economics, Analytics, Compliance, Coding, Post-Acute Care, Network, and Payer Relations to drive program alignment and progress.

Track program performance against defined KPIs, quality metrics, and operational goals; escalate risks and barriers as needed.

Partner with analytics teams to support development, validation, and maintenance of dashboards and routine performance reports.

Facilitate regular program meetings, prepare agendas, document decisions, and ensure follow-up on action items.

Support gap closure initiatives related to preventive care, chronic disease management, and utilization management.

Assist in the implementation of pilot programs and scaling of successful population health interventions.

Monitor compliance with regulatory and payer requirements, including CMS, ACO, and NCQA standards, and support audit readiness activities.

Identify process improvement opportunities and support execution of continuous improvement initiatives using data and stakeholder feedback.

Assist in identifying performance gaps through data analysis and support the implementation of targeted interventions.

Maintain program documentation, workflows, and standard operating procedures.

Support change management efforts, including communication planning, stakeholder engagement, training coordination, and adoption of new workflows.

Assist with resource planning and coordination to ensure programs are adequately staffed and supported.

Prepare summaries, reports, and presentations for leadership review.

Stay informed on population health best practices, regulatory updates, and industry trends.

Foster strong working relationships across departments to support collaboration and program success.

What You Need:
Education
Bachelor's Degree Bachelor?��s degree in healthcare administration, public health, business administration, nursing, social work, or related fields Relevant work experience may be considered in lieu of a degree. Pref
Experience
5 Years Experience in post-acute care, care coordination, population health, value-based care, or payor, managed care, or related environments. Demonstrated experience in project coordination or program management, quality or performance improvement. Req
Licenses and Certifications
CAPM, LSS, CPHQ, or other project management training/certification preferred. Upon Hire Pref
Skills
Experience using project management methodologies overseeing project portfolios.
Strong written and verbal communication skills; able to concisely present information required to drive alignment and decision-making.
Ability to connect program activities to broader organizational and value-based care strategy.
Analytical skills to guide, manipulate and analyze relevant data to inform sound decision-making.
Comfort working with dashboards, BI tools (e.g., Tableau), and population health platforms.
Proficiency in Microsoft Office Suite, including Excel, PowerPoint, and Word and experience using data and presentation tools to support analysis, reporting, and executive communication.
