The ideal candidate will possess the following qualifications:

•  Bachelor's Degree in Nursing, Social Work or other related healthcare degree required.

•  3 Years of experience healthcare related experience to include experience in managed care and physician practice setting, experience in and expert knowledge of continuum of care (transitions of care) and one year of experience in accountable care organization and/or risk adjustment programs required And,

•  1 Year of experience Supervisory experience or 1 year experience in a leadership role (preceptor, process improvement facilitator, clinical educator) required.

•  RN – Registered Nurse upon hire required or,

•  LMSW - Licensed Master Social Worker upon hire required or

•  Other clinical discipline upon hire required.  

 


Skills:

• Knowledge of Care Coordination, Discharge Planning, and/or Case Management within an ACO Environment.

• Knowledge of the principles and methods to restore or enhance social, psychosocial or bio psychosocial functioning of individuals, couples, families, groups, organizations and communities. Interpersonal skills to interact with a wide range of constituencies, verbal and written communication skills.

• Knowledge of criteria for continuum of care levels of care and services
