Position Summary

The Director of Population Health Strategy provides enterprise leadership for the design, governance, and optimization of population health and value-based care programs. This role translates organizational strategy into scalable initiatives that improve quality, patient experience, utilization, and total cost of care. The Director partners across Quality, Care Management, Risk Adjustment, Pharmacy, Analytics, and Payer Relations to ensure population health programs are effectively designed, executed, measured, and continuously improved.

Position Duties

Lead the end-to-end lifecycle of population health initiatives, from concept and design through implementation and optimization.

Translate enterprise value-based care strategy into actionable population health programs.

Evaluate population health performance within value-based contracts and identify opportunities to improve outcomes and financial results.

Identify, pilot, and scale innovative care delivery and performance-based models.

Establish KPIs, quality improvement standards, and performance metrics aligned with enterprise goals.

Partner with analytics teams to develop dashboards and executive-level reporting.

Drive continuous improvement using data analysis, audits, and structured feedback loops.

Present population health performance results and strategic recommendations to executive leadership.

Collaborate closely with Quality, Care Management, Risk Adjustment, Pharmacy, Compliance, Coding, and Payer Relations teams.

Support gap closure strategies across preventive care, chronic disease management, and utilization.

Apply change management, risk management, and resource planning principles to support program execution.

Ensure population health programs comply with CMS, ACO, NCQA, and payer requirements.

Remain current on regulatory changes, population health trends, and quality program requirements.

Promote patient-centered, equitable care models across the continuum, including attention to access, experience, and outcomes.

Lead implementation planning, including timelines, milestones, and risk mitigation strategies.

Hire, develop, and retain high-performing population health and program teams.

Foster a culture of accountability, collaboration, and continuous improvement.

Education
Bachelor's Degree in Healthcare Administration, Public Health, Nursing, Social Work, or related field required
Master's Degree in Healthcare Administration, Public Health, Nursing, Social Work, or related field preferred

Experience
8 years progressive experience in population health, value-based care, or managed care environments to include people management experience required

Licenses and Certifications
RN, or, LCSW, or, LMSW required upon hire
PMP - Project Management Professional Certification preferred upon hire
SSLP - Six Sigma Lean Professional preferred upon hire

People Management - Provides oversight and direction of department leaders in the areas of staffing, talent development, performance management, and resource planning to support strategic goals.
Strategy Execution - Translates organizational strategy into actionable department plans, aligning goals and operations to support long-term success and cross-functional coordination; ensures vision and strategy is clearly communicated to all staff.
Performance & Accountability - Owns department performance by setting clear expectations aligned to strategy, using data to measure and monitor results, identifying risks, generating solutions to improve outcomes and making decisions within departmental scope and escalating when appropriate.
Change Leadership & Operational Readiness - Design change initiatives by implementing new technologies and processes, lead teams through transition, ensuring resources are aligned for success and proactively communicating the purpose and benefit to build engagement.
Strategic Relationship Management - Builds and maintains strategic relationships across departments, with senior leadership, and with external partners, ensuring clear communication, alignment, situational transparency related to business priorities or emerging issues, and accountability for performance expectations.
Financial Resources & Stewardship - Oversees departmental budget and resource planning, ensuring effective use of funds, identifying opportunities for operational efficiency and continuous improvement and making informed decisions to balance competing initiatives and optimize department outcomes.
